WASHINGTON -- The U. S. Food and Drug Administration announced Tuesday that it will require the labeling of its arthritis drugs Celebrex and Vioxx, which both contain the arthritis drug naproxen, to show that they are "not associated with serious adverse effects."
Celebrex and Vioxx are a new class of anti-inflammatory drugs that the FDA has approved for use in treating pain and other conditions that patients may be more likely to experience after taking them. The drugs were approved by the FDA in May for the use of the older pain relievers, naproxen and ibuprofen.
The new labeling for the arthritis drugs will not include an boxed warning on all drugs, and will include information on the risks associated with the use of these drugs. The new warning on naproxen and ibuprofen has not been announced yet.
"This information is important for anyone considering using these drugs as part of their daily treatment plan," said Dr. Sidney Wolfe, director of the Public Citizen Center for Drug Safety. "The FDA is committed to ensuring that patients receive safe and effective treatment with the most effective and most appropriate treatment options for their condition."
Celebrex, which contains Celebrex and Vioxx, has been linked to an increased risk of heart attacks and stroke, which can be fatal. The FDA has suspended the use of Celebrex in some patients, citing the risk of a fatal heart attack or stroke.
Vioxx, sold under the brand name Zoloft, also has an increased risk of heart attacks and strokes. The FDA has asked the makers of the two drugs to clarify whether they should list their warnings on all drugs, including Celebrex.
"We believe it is important for patients to report any unusual or worsening symptoms to the FDA's Committee on Safety," said Dr. Charles W. Wolfe, executive director of the Public Citizen Center for Drug Safety. "We hope that they will take the time to fully educate the public about the risks and benefits associated with these drugs."
The new labeling is based on the agency's latest study on naproxen and ibuprofen in animals and is designed to help patients understand the risks associated with the use of these drugs.
The study used a population of animals in the United States, as part of the National Institutes of Health (NIH) National Library of Medicine, and found that naproxen and ibuprofen are generally safe for use in humans.
While some drugs are not shown to cause serious side effects, such as gastrointestinal problems, the risk of stomach ulcers, stroke, heart attack or heart failure appears to be higher for Celebrex and Vioxx than for naproxen and ibuprofen. In 2004, the FDA reported that Celebrex and Vioxx were linked to an increased risk of heart attacks, a heart attack in people 65 and older, and death from heart failure in people younger than 60.
The FDA said it was concerned that such data were lacking in animals, and that it was not clear why Celebrex and Vioxx were linked to such increased risk.
"I'm concerned about the increased risk of serious adverse effects associated with this drug, particularly when considering people with other chronic conditions," said Wolfe. "These drugs can be harmful in animals and patients at higher risk for serious adverse effects."
Dr. James L. Wexler, the chief of the division of endocrinology at the Mayo Clinic in Rochester, Minn., said, "The increased risk of serious adverse effects has not been adequately studied, and the Food and Drug Administration is working with the NIH to increase the labeling of all of their drugs."
The FDA has not responded to questions from the New England Journal of Medicine about the study, but has indicated that it plans to continue working to ensure that patients are aware of the risks and benefits of these drugs.
The FDA has also issued guidance to doctors and patients that they should ask questions about the safety of Celebrex and Vioxx, along with other medicines.

In some cases, patients are taking other medicines that can lower the dose. These include ibuprofen, aspirin, diclofenac, aspirin, and warfarin.
Celebrex also has a weak and relatively strong interaction with other NSAIDs such as ibuprofen. The risk of bleeding is higher in people with a history of gastrointestinal bleeding or who have had a previous stomach ulcer or perforation while taking celecoxib.
